The type of personal information we collect
We currently collect and process the following information:
  • Your name, date of birth, address, and NHS Hospital ID number
  • telephone number only if we need to contact you, or if you contact us via telephone
  • Email address if you contact us via email
  • Your registered GP
  • Necessary clinical information such a diagnoses, medical history, medication and investigation results to make decisions or recommendation about your care

  • How we get the personal information and why we have it
    Apart from the information about you that you share with us directly (e.g. telephone number or email address) we receive personal information indirectly, from the following sources in the following scenarios:
  • your General practitioner and other healthcare professionals working for them, and the records they keep about you, when they refer you to us, or when we require clinical information to make decisions or give advice about your care.
  • Your NHS Hospital records (at the Isle of Wight Healthcare Trust) for personal details as well as required clinical information to make decisions about your care
  • other registered healthcare professionals we work with to provide shared care, e.g. surgical colleagues when they ask us to assist in your care, e.g. anaesthesia services.
  • Secretarial staff working at The Mottistone, St. Mary’s Hospital
  • Other private or public healthcare providers or companies referring you to us to provide clinical advice or care.

  • We use the above information in order to
  • Contact you to book appointments and respond to your enquiries
  • provide private medical services, making decisions about your care
  • Make information about your assessment, medical advice and treatments available to you
  • Send you bills and process payments

  • We may share this information with
  • your registered General practitioner or healthcare staff working for them
  • Registered private or public NHS medical colleagues providing shared care for you
  • Companies or other entities having referred you to us (these will have obtained your consent prior to referral)
  • NHS secretarial and administrative staff
  • Public authorities in certain situations if required by law.
  • your private healthcare insurer, if you have insurance

  • Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:
  • Provision of private healthcare including billing
  • Your consent. You are able to remove your consent at any time. You can do this by contacting us (details at the top)
  • We have a contractual obligation to our partners
  • We have a legal obligation
  • We have a legitimate interest

  • How we store your personal information
    Your information is securely stored on local electronic devices and letters/ documents we receive from referrers.
    We also store encrypted electronic backup copies on external computers using GDPR-compliant business services.

    Paper records we receive about you are disposed of safely by shredding.
    We keep the above listed information about you as long as we are required by current UK law (Healthcare records). We will then dispose your information by deleting your electronic file from our current database as well as from externally stored copies of our database.

    Your data protection rights
    Under data protection law, you have rights including:
    Your right of access - You have the right to ask us for copies of your personal information.
    Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
    Your right to erasure - You have the right to ask us to erase personal information in certain circumstances, unless we are required by law to keep certain records e.g related to health and care.
    Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances. Please note that we are lawfully required to share certain types of clinical information with UK authorities.
    Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.
    Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
    You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.

    How to complain
    If you have any concerns about our use of your personal information, you can make a complaint to us at our above listed address.
    You can also complain to the ICO if you are unhappy with how we have used your data.
